Validate Your Identity

1. Enter your providerConnect™ Provider Number if you are an Extended Health Services Provider, or if you are a Drug Provider, enter either your CPhA/ ODB / AQPP Number or your providerConnect™ Provider Number.
2. Enter the name of the practitioner exactly as it is shown on your most recent statement (i.e. DR J DOE (no punctuation) or ONTARIO 12345 LTD).
3. Click 'Continue'.
4. You will be required to answer your challenge question.
5. Click 'Submit'.
6. Confirm your Email Address by re-entering it.
7. Your Username will be forwarded to your Email Address.

Sign In

1. Going forward, you will go to the Sign In area on the Secure Services Welcome Page and enter your Username and Password to access your account.
